Background of the Study

Healthcare financing plays a critical role in the accessibility and quality of healthcare services, particularly in developing countries where financial constraints are often a barrier to the effective delivery of health services (Okereke et al., 2023). Primary healthcare (PHC) is considered the cornerstone of healthcare delivery in Nigeria, aiming to provide essential services that are accessible, affordable, and effective for the entire population (Adebayo et al., 2024). In Kogi State, located in central Nigeria, the healthcare system faces several challenges, including inadequate funding, poor infrastructure, and a shortage of healthcare workers, which significantly hampers the utilization of primary healthcare services (Adamu et al., 2023).

The Nigerian government has introduced various financing mechanisms aimed at improving access to healthcare services, particularly in rural areas, where the majority of the population resides (Okoro et al., 2024). These financing mechanisms include the National Health Insurance Scheme (NHIS), the state government’s health funds, and out-of-pocket payments. However, the effectiveness of these financing mechanisms in increasing the utilization of primary healthcare services in Kogi State has not been adequately assessed. Evidence from other regions of Nigeria suggests that inadequate healthcare financing and improper allocation of resources contribute to the underutilization of PHC services, especially in underserved and rural areas (Aliyu & Adeoye, 2024).

This study aims to evaluate the effect of healthcare financing on the utilization of primary healthcare services in Kogi State. By examining how different financing mechanisms influence the availability, accessibility, and quality of PHC services, the study will provide recommendations for improving healthcare delivery in the state.

Statement of the Problem

In Kogi State, there is a significant gap between the availability of primary healthcare services and their utilization by the population. Despite government efforts to improve healthcare financing, many residents, particularly in rural areas, face barriers to accessing primary healthcare services. These barriers include financial constraints, inadequate healthcare infrastructure, and insufficient awareness of available services. The study seeks to investigate the role of healthcare financing in improving the utilization of PHC services in Kogi State.

Objectives of the Study

1. To assess the relationship between healthcare financing and the utilization of primary healthcare services in Kogi State.

2. To identify the challenges faced in the allocation and management of healthcare financing in Kogi State.

3. To evaluate the impact of different healthcare financing models (e.g., NHIS, state health funds, out-of-pocket payments) on the accessibility of primary healthcare services in Kogi State.

Research Questions

1. What is the effect of healthcare financing on the utilization of primary healthcare services in Kogi State?

2. What are the challenges associated with the allocation and management of healthcare financing in Kogi State?

3. How do different healthcare financing mechanisms impact the accessibility and utilization of primary healthcare services in Kogi State?

Research Hypotheses

1. Increased healthcare financing is positively correlated with the higher utilization of primary healthcare services in Kogi State.

2. Poor management and allocation of healthcare financing resources negatively affect the utilization of primary healthcare services in Kogi State.

3. Health financing models such as the National Health Insurance Scheme (NHIS) and state health funds increase access to primary healthcare services compared to out-of-pocket payments in Kogi State.

Scope and Limitations of the Study

This study will focus on the effect of healthcare financing on the utilization of primary healthcare services in Kogi State, using data from healthcare facilities, policymakers, and community members. It will explore various financing models such as NHIS, state health funds, and out-of-pocket expenditures. Limitations include potential bias in reporting by healthcare workers, challenges in data collection from remote areas, and difficulty in obtaining accurate financial data from health institutions.

• Healthcare Financing: The process of providing funds for healthcare services, which may include government funding, health insurance schemes, and private payments.

• Primary Healthcare Services: Basic health services that are essential to the well-being of the population, including maternal and child health, immunizations, and treatment of common illnesses.

• National Health Insurance Scheme (NHIS): A government-funded health insurance program in Nigeria aimed at providing access to healthcare for all citizens.

• Out-of-Pocket Payments: Direct payments made by individuals at the point of service for healthcare services, without insurance coverage.
